I would like to find out how do hospitals know who is employed or selfemployed and therefore bill them?

The hospitals that fall under a national health system charge patients according to employment and non employment. My question is what tools do these hospitals have tosee that this person is employed or self employed or not. Or even whether the person is relly unemployed.

  1. The hospital personnel who attends the patient will be asking the bystander or the persons bringing the patients who at that moment will be in another frame of mind all these questions.They will never cross check and bills in the way that is most profitable for them.
