How to get Medical Records from Mercy Hospitals in Chicago?

I'm 15 years old and i need copies of my medical records for educational purposes, and i need them before April. I want to know: 1. Where can i recieve copies of my Medical Records? 2. If i must go to a Mercy Hospital to retrieve them, do i have to bring a parent/guardian? 3. Are there any fees? Thank you!

1. I just googled it. Call them and ask them about fees and such. 2. Your going to have to call the medical records department at the hospital. They will tell you how much since it is not for a doctor they are going to most likely charge you each hospital is different. You will need a parent since you are a minor they will probally need to call etc. It takes some times a few weeks to get things copied and of course they might ask why you need them. Your parents will have to sign a release form for the records.
