How were patients treated in the old insane asylums?

1. Depending on what you are defining as "old". Large institutions housing mentally ill varied greatly. Some were not much more than custodial institutions. In some issues of abuse and maltreatment were reported. Many attempted to provide treatment regimes to try and rehab patients and return them to society. Most large institutions where closed in favor or a community approach to community health.
