New York Hospital Records: How can patient request, what can I request, what rights do I have?

Why do NY hospitals make it so difficult for patients to get patient's own records? Is there a way to get a summary after each doctors visit? How long does a hospital have to legally keep patients records before they are destroyed or stored forever with no access by patient? A hospital tried to charge me over $400 for my own patient records. Is this legal? They are charging .75 cents per copy. Also, when I request patient records how long do I have to wait before the hospital must provide the records? Thanks.
